I play the game on PS2 format and the order i done it in was
100% easy
100% medium
100% hard
then Unleash the fury mode popped up when i went to start a new game.
(But i am not sure if u just have to complete hard 100%)

If they made the game a bit more like GTA SA i reckon it would improve it

hmm depends where the fight takes place and also depends on weather the Bulls are allowed to use weapons.
